KHEO 2021 Annual Open Enrollment

Open enrollment for eligible employees begins now through Monday, November 30th.

During This Time You May:

  • Review Your Benefit Options
  • Enroll For First Time
  • Workplace Violence: The Early Warning Signs – Employee Version
  • Make Plan Changes
  • Add/remove Dependents
  • Decline Coverage

All elections made during this time will be effective January 1, 2021. Enrollments or changes are binding for the entire year, unless you experience a Qualifying Event. Completed paperwork may be submitted via email benefits@mmchr.com or fax (310) 360-5100.
